NHS 111 services are available for advice and guidance to patients.



New Patient Registration

New Patient Registration requirements:

  1. A Medical Card if previously registered with another doctor in the country.
  2. An FP1 registration form to be provided by the practice if never been registered in the country
  3. Where possible please provide a form of ID to avoid any spelling mistakes and a document to prove that you live in our catchment area.
  4. All registered patients are allocated a named GP

New patients are booked for a general health check-up with the nurse

Please note: We register patients for access to primary care services. If you need secondary care services (hospital referral), the hospital may independently assess you for eligibility to such services.


The post codes covered by our surgery mostly include: NW10, NW2, HA9, HA0.
